Start with the conclusion
Carpal hyperextension means the carpus collapses into abnormal palmar flexion. Management depends on the cause: fractures and dislocations usually need surgical stabilisation, while injuries to the supporting ligaments (palmar ligaments and palmar fibrocartilage) can be managed conservatively in some cases with a supportive splint or brace, activity restriction and rehabilitation.
Signs that need urgent care
- Obvious deformity of the carpus after trauma, with inability to bear weight.
- Open wounds or bleeding.
- Sudden onset with severe pain.
- Cold or pale distal limb.
With any of these, seek veterinary care immediately; do not attempt home splinting or massage.
Why hyperextension happens
Carpal stability depends on multiple bones, ligaments, a joint disc and the palmar fibrocartilage. When the palmar supporting structures are damaged — jumping from a height, a sprain, or chronic overload — the joint can no longer hold its normal angle and collapses. Common causes fall into three groups:
- Traumatic: ligament tear, fracture, dislocation (jumping, falls, road accidents).
- Immune-mediated: part of a polyarthropathy, which needs medical assessment.
- Secondary: long-term overload or structural abnormality, including breed-related carpal problems.
Why diagnosis matters
Radiographs of the bones are the baseline. When ligament injury is suspected, stress or weight-bearing views, or CT, may be needed to judge the collapse angle and location. Different causes mean completely different routes — this is what decides surgery versus conservative care.
Treatment paths
Surgical direction
- Fractures and dislocations: internal or external fixation to restore structure.
- Severe, complete ligament rupture: partial or full pancarpal arthrodesis is commonly used to restore support by stabilising the joint; the literature treats it as an established surgical approach to carpal hyperextension.
Conservative direction
Where the injury is milder, or where surgery is not suitable for the dog or the owner, conservative management usually includes:
- A supportive splint or brace to support the palmar aspect and limit over-extension.
- Strict activity restriction: lead walks, non-slip surfaces, no jumping or sharp turns.
- Rehabilitation: progressive loading and proprioceptive work under a rehabilitation veterinarian.
- Scheduled rechecks: collapse angle, skin condition and functional change.
Where a brace fits in these cases
- Role: external support remains one of the core elements of conservative management — limiting motion in the direction of hyperextension and helping some cases reload.
- Limit: a brace cannot repair a torn structure; if the underlying problem is a fracture or a major dislocation, it cannot replace surgery.
- Risk: the carpus has prominent bony points and thin soft-tissue cover, so pressure injury risk is comparatively high; measurement, moulding and rechecks must be handled by a professional.
What an owner can do now
- Establish the cause through imaging before discussing a plan.
- If conservative management is chosen, ask what direction the support works in, how often to recheck, and how to inspect the skin.
- Check the skin and toes daily: redness, broken skin, dampness and odour should all be recorded.
- Do not re-heat, re-mould or modify the device yourself.
